Insurance and Billing Information
When arriving for your first visit, please provide proof of insurance. If
you are a member of HMO insurance, make sure you receive proper authorization from your primary care physician, prior to arriving for your appointment.
Our doctors are Medicare participating physicians and will accept assignment on these claims after the deductible of each year is met.
It is important to notify us if you have a supplemental insurance policy. We accept Medicare assignment with a supplemental policy after both deductibles are met. You are not billed until after the insurance carriers have paid their share of your bills. Then we will send you a descriptive statement of services given and balance due.

The Patient Accounts Department is located at the Downtown office. Our
Representatives specialize in cardiology coding and billing. They have
over 10 years experience in claim filing, Medicare and managed care.
They continue to stay updated at all times with new laws pertaining to
Cardiology, Electrophysiology and Peripheral issues. Should you need to
speak to a Representative in the Billing Department, you may call 727-329-1600.
