Procedure for First Time Patients
The Heart and Vascular Institute makes every attempt to schedule appointments for new patients as soon as possible, generally within two weeks. We require your medical records from your Primary Physician, who will usually mail or fax the records to us prior to your scheduled appointment. After your first appointment you will be mailed a New Patient Form to complete and bring with you to your appointment. This form also contains a map to our offices. We will call you to confirm your appointment and verify insurance. Should you have insurance requiring pre-authorization for your visit, we will call your Primary Physician or the Insurance Company directly to obtain proper authorization. When you arrive at our office, we will make a copy of your insurance card for your file. We are required to collect any necessary Co-Payments.

After your first appointment has been scheduled, you will be sent a "Medical History Form" and an "Insurance Information Form". This information is very important for a comprehensive cardiovascular evaluation because it includes the symptoms of heart disease and related systems as well as the "risk factors" for the development of atherosclerosis (high blood pressure, diabetes, high cholesterol levels, smoking and family history). For a complete evaluation, we should have records of prior heart or vascular surgeries or other pertinent tests (prior cardiac catheterization, echocardiogram, Holter monitor, cholesterol profile and other laboratory work-up and a chest X-ray).

Should you call and have an emergency such as chest pain, a Triage Nurse will immediately assess your call. Accommodations are made for emergency visits. However, you may be referred directly to the Emergency Room.
